R Medial Knee Pain

Hello everybody. I am hoping someone can provide me with some info regarding my R knee. I have a dull, hot, tweaky, pain on the inside of my knee that is become a nag. I have attempted to run on it, and usually can do about 4 clips, but afterward I have this pain that has forced me at this point to stop. Something is just not right. Today is the fourth day without running and I think it is slowly getting better. I am an experianced runner (2 Marathons) that was putting in about 25 miles per week. As is always the case, this pain surfaced after adding miles to my runs. It was a bit of a drastic jump. I think I went as high as 35 miles on back to back weeks when I noticed this pain. My question is: Does anyone know what is going on with my knee? A sprain? A ligament tear (God forbid)? Tendinitos? Also, can I ride a bike while I take a break from running? Any advice would be appreciated.

My guess is an irritation of the pes anserinus tendon (it attaches to the end of the tibia just below your knee cap on the medial side), or pes anserinus bursitis (the bursa in your knee by this tendon). My advice for either is to stay away from downhills, cut back on your miles, ice,... common sense.

I too just recently discovered the exact same pain. I was training for a marathon and with 2 miles left of my 15 miler on a sunday i developed a sharp pain on my right inner knee. As i got home and sat around it got worse. So bad i could not even walk up or down stairs without aide. I took a half week off and biked instead. I started up with a few small runs and on the second day it was back. This past week I took a complete whole week off with absolutely no exercise and pounded abti inflamatory pills took glucosamean and today I ran a 2 miler and by the end my knee was killing me. My marathon is at the end of July and if I took a whole week off with absolutely no improvement I am feared that all my training is down the tubes. PLEASE PLEASE PLEASE someone have some insight on this for us and anyone else out there with similar problems. Thanks

Mine was left inner knee. Did the rest, ice, etc., nothing helped. Finally went to a sports medicine doc. After he ruled out anything serious it ended up being bursitis. No fun at all. Constant aching in that one area. Got a cortizone shot and things are good. I still ice once in awhile and am very careful when I run on uneven type terrain. Good Luck...

Sounds similar to medial collateral ligament sprain. I beat it after 5/6 months while continuing to run. But you have to do quad and ham stretching/strengthening exercises that your physiotherapist will recommend if it's MCL.

I'm in PT right now for pes anserinus tendon and pes anserinus bursitis (the bursa in your knee by this tendon). PT has me strengthening quads, hams...I do dead lifts,ham machine, squats,step ups,jane fondas. I will get orthodics and new shoes. I'm having some issues with ITBS also. But, the inside knee seems to be doing better and getting stronger. I also started ART message and therapist stretches out those 3 tendons that come together on inside knee. After 1 session with her I saw a big difference. Maybe you should checkout your shoes first.

I agree that it could be medial collateral ligament sprain. Maybe best to check with sports doc.

These are only things I'm working on for these problems.

I've had both an medial collateral ligament sprain (left knee) and pes anserinus tendinitis/bursitis (right knee). They feel -- are -- very different. Spraini was the result of a specific trauma. In my case, collision in soccer. Sprains are not overuse injuries. With a sprain there is point tenderness and achiness, but not the recurring sharp or stabbing pain of bursitis. With a sprain, there is more likely to be discoloration and swelling and a general feeling of achiness and instability. In my non-professional opinion, an mcl sprain would be an extremely remote possibility for a running injury. Still it is possible that you could have twisted your knee badly enough to injure yourself if you were runnng on a bumpy trail or something. But you would have known you did something bad the instant you did it. (My mcl sprain trauma was the most painful thing I've ever felt in my life, including childbirth without pain meds.)
Pes anserinus bursitis and pes anserinus tendinitis are typical chronic overuse injuries... both are much more likely candidates for what is ailing you. ... just my opinion.

I wish I could help, but I can only empathize with your desperation. So many of us here in the med tent understand what it is like to face the possibility--or reality--of not being able to run like we thought we could, if at all!

Don't lose hope, and learn from those who have been beset by injuries for years, and keep coming back. It will require a much more flexible mindset, the willingness to let go of goals temporarily until you take care of the body.

It can also be a lot of letting go of that desire to be in shape, as well as all those fabulous things we get from running.

Also, pray hard. It can change things for you.

Hope you find the problem. Hope they give you an MRI. Fight for it. It is the only real help in diagnosing knee stuff

Hi there,

I am not a doctor but I am a triathlete who experienced a similar issue last year. After increasing my mileage a bit too quickly, I had pain on the inside of my knee. It would start off as a dull ache and then finally be so sharp that it would force me to stop running.

I was diagnosed with plica band syndrome. There is a "spandex" like band that wraps around the inside of your knee. It is normally pliable but when overused (due to a weak medialis in the quad) it become taught when the knee doesn't track right. The thigh bone then "catches" on it causing the sharp pain. This may NOT be what's happening to you but it's a possibility. Regardless, I would get thyself to a sports doctor ASAP.

Recovery for me consisted of a cortinsone shot (liquid magic) then PT for several weeks. No running...lots of strength training the inner quad and lots and lots of stretching particular the hip flexors and hamstrings. As for biking...flats are fine but no hills. I upped my volume in swimming, started water running and using the elliptical to maintain cardio fitness.

Hope this helps...but do check in with a doc!
