knee pain I can't diagnose


i used to run over 40 miles a week, then went down to only 24 miles a week because i took up cycling as well. i added taking my dogs and pushing my son to my running as well. i made all these changes 10 months ago.

when i began running a few years ago, my left knee gave me problem because the petella tendon had a crappy connection to my tibia, but as i eased into running i built up it strength and it is good. but now after years of running and 10 months after dropping my miles, my right knee which has alway been the strong knee, hurt when i do any extension motion of the lower leg. the pain is on both sides and slightly behind the patella down to just below it.

the only things i can think of that happened around the same time as the pain in my knee was i had just gotten over some nasty anterior shin splints in both legs that i ran on even though i knew i shouldn't have, and then i stopped running on concrete (the worst surface for knees) in a hilly neighbor hood in western washinton, and started running a pavement (slightly better than concrete) that is basically flat.

i know my shoes are not wrong for my foot, since i have been running in the same shoe style for a year and a half, however i have also never let my shoes get as worn out as my current two pair are (i have a new pair that should be delivered tomorrow).

so if anybody has any ideas what the knee injury is, and any ideas what might be causing it, i would appreciate your input. i have tried to include all the information i could think of. but if you have any questions i didn't answer, i will be happy to answer them, since you are doing me huge favor. i don't want to go to the doctor for this because i am trying to get into the army, and i am already needing to battle for a medical waiver because of a back surgery, so a problem with my knee would close the door on my chances. and just incase my age helps to diagnose my knee problem i am 24.

I'm wondering whether your knees are not tracking correctly when you cycle. Since cycling is a big change, that's what I'd look at first. Do you use clipless pedals (clip-in)? Sometimes all you need is a slight adjustment to the angle of your cleat to straighten everything out. Then I'd also look at seat height and play with that. I would also avoid running downhill and avoid hard climbs on the bike until this eases up. Frequent icing and gentle stretching (quads and IT band) will also help.
