I had my surgery done in Boston--Dr. Peter Asnis out of MGH. He is one of the doctors who works with the Patriots and the Red Sox. I have been very impressed with his honesty, professionalism and he didn't just do the surgery and forget about me. I saw him 2 weeks post-op-he came to see me at PT 4 weeks post-op, I had an appt 6 weeks post-op, 3 months post-op and I have another appt scheduled for 6 months post-op. He is young-and trained at the Hospital for Special Surgery in NY. I was impressed by his resume which you can see if you go to MGH website. I have continued with PT at MGH--and the facilities are right across the hall from the surgeon's offices on Cambridge Street, around the corner from MGH. I had hip arthroscopy / femoral neck debridement by an orthopedic at SCOI (Guanch-Van Nuys) in May '07. During my pre-op appointment, I asked about recovery and ability to run again and was told I would be back to my old routine within 6-8 months. I am no where near recovery. I was on crutches for 4 weeks. I never missed PT. Initially, I was optimistic in my recovery. I went in for my 8-10 week follow up and asked if I could start testing my hip. I was given the okay. I worked out in the sand (I live by beach) and everything felt great. I than ran for about 1/2 mile on the pavement (I used a GPS and really monitored by pace). I felt great right after the run....but later in the evening I had different pain. I rested for about 10 days and tried a slow PT regimen. I went back to see my orthopedic and initially he thought I was a new patient coming in for a consult (he didn't even know who I was--never looked at my chart prior to coming into the room??). He had a fellow with him and started to ask how I was doing once I told him who I was. To make a long story short, he basically said I might not get back to normal. After reading many posts on the topic, I realize there are a lot more patients that do not fully recover. My pain in my hip is not the same as before surgery (groin & charlie horse pain) but now a new pain that feels like it is more on my side (bone / tightness). I am VERY disappointed with my results. Had I known that the chances for failure were real, I would have delayed or done more research (maybe dr phillapon in Vail with more experience / second opinion from a hip arthroscopy specialist). I can exercise on a stationary bike, swim a bit but the activities I loved (running, surfing, tennis)--I clearly can not do anymore. My experience has been miserable. I am now 8 months post op and running, surfing, tennis seems like an eternity (at best) away. I am empathetic to your husband.

That's all good to know. I experience the groin pain along with the charlie horse pain(involves several phrases and words a lady shouldn't be saying) when I ride horses(starts at hip goes all the way down to my knee). The last doctor I saw has changed his mind and said I have arthritis(I know that), not a labral tear, and to wait 3 months and take presciption anti-inflammatories. Also I need to modify what I do. Told him, sorry, that's not going to happen. It's bad enough that I had to leave my job because I was physically unable to do it(9 hours on my feet, extensive amount of walking around). Was denied unemployment because my employer fought it, made up some stuff and won. I am paying for my health insurance out of my own pocket and it's not the state subsidized insurance so it's costing me a fortune. And am having trouble finding something I can do because of lack of training and no money to get schooling. So this is all dismaying for me, especially if further surgery may only make me worse in a different way.

My question is, How can one doctor say that I have a labral tear and the other one says that I have arthritis? Especially since the other one also said I've got bone rubbing on bone in the joint. Just venting but I certainly don't want to feel any worse than I already do.

I understand your frustration. First of all-have they done an MRI with contrast yet? It's very difficult to see a tear without this test. Secondly, I had x-rays, the MRI with contrast and we still went into surgery not knowing the extent of the damage. My x-ray showed 'signs' of arthritis and my MRI showed some arthritis and it was'suspicious' for a labral tear. When my surgeon went in-he found the labrum shredded and he found extensive arthritis in that hip joint. Because the joint is so deep, it is difficult to see clearly on x-ray or MRI......I am now over 4 months post-op and unfortunately I am still having a lot of pain. I am still going to PT but I am realistic to know that this may be as good as it gets. I am grateful that I had the surgery--it gave me the chance to know exactly what was going on inside instead of speculating. I do have some improvements but there is nothing you can do about the bone on bone pain of arthritis......Make sure you are going to a reputable doctor. I think you said you are near Boston--I was pleased with my surgeon, Dr. Asnis-have you tried seeing him? Good luck. Sue

I just wanted to post an update on my hip arthroscopy surgery. Seems as though many of the latest posts are less-than-optimistic post-surgery, so I wanted to share my story since mine is good. I'm 6 months post surgery. I had a bone spur ground down, labral tear repaired and some hardened cartilage removed. With all that fun stuff going on, I was on crutches for 6 weeks.

I am running two days a week and/or rowing two days a week, filling in the rest of the week with the elliptical, stair climber, step mill, and weights. I also get out and hike on the weekends when weather permits. So far, the longest I've been able to hike is about 9 miles. I don't really have any pain. Initially, I did have a lot of pain in my hip flexor, but my surgeon reminded me that I was just breaking up scar tissue. After the first few weeks, the hip flexor pain stopped. Very rarely do I feel achy or pain. I used to have extreme hamstring pain from overcompensating for the injury. My hamstring might occasionally act up a bit, but it's way better than it used to be.

My next follow-up appointment is at the end of February, which will be 7 months out. My surgeon, Dr. Waslewski, Scottsdale, AZ, says that the tear repair should be healed by then, but the bone spur can take up to 1 year to heal completely. I'm pleased with my results. However, I don't think I can stress this enough -- listen to your doc and listen to your physical therapist. I was on crutches for 6 weeks . . . hated every minute of it. But I did everything the doc and PT said. My PT thinks this is why I've done so well and had so little pain. It's easy to think you can do more -- even now I want to do more than 2 days a week of running, but I don't want to mess anything up, so I'm sticking to the doc's recommendation. It's definitely one of the best things I decided to do.

How did you surgery go? How are you feeling?

I just had my other hip done by Dr. Kelly. I'm feeling better then after the first surgery. I'm not sure how successful the first surgery or recovery was. I hope it will be better the second time around.

Definitely cant say enough about doing your research and finding a dr who has done A LOT of labral repairs, and unfortunately, there aren't a lot that do. It is so new, most hip drs just clear the area instead of repairing. Philapon is super expensive and amazing, but as an alternative try to look for some of his old fellows (like my dr, King in st louis) who trained with him. I am still feeling great and am so glad I had the surgery. (Also, you MRI's wtih contrast are the best way to diagnose the tears)

Hi Kristina,

I am doing pretty well. I ended up having my labral tear debrieded, my psoas released, and I had a really severe case of synivitious. My surgery was on January 15th and I am still on crutches. I was doing pretty well pain wise until I started going to PT at week two and all of the banding and stretching my pt did to my leg and hip has caused the pain to creep up. We are keeping an eye on that cause we don't want the synivitous to come back. I am not having two many problems with the psoas, just mild tightness. Most of my pain is in my butt and lateral hip (side pain). How are you doing? When did Dr. Kelly do your second hip?

hi all!

i just read laura's last post and i'm terrified at what i think the correct answer to my question is:

i was told i had a pretty nasty labral tear last year when i was training for my first triathlon. i had my heart set on completing it and pushed through anyway. since then, i have done two triathlons on my torn hip. i got through both races and while the recovery was a little longer than most athletes, i felt for being injured i was back it rather quickly. i know i'm not supposed to run but at this point in my life, i can't justify having experimental surgery and not being active, plus i'm not sure my insurance would even cover it.

all in all- i am thinking about running my first marathon (without surgery to repair my tear). is this a crazy idea? i have been able to complete 10ks just fine and while obviously 26.2 miles is much different than a 10k, i am grossly optimistic. i know that i can mentally get there, but my concern is if my body can physically endure this? any and all advice would be greatly appreciated!!!

Dani,

Be warned: I would not advise you to undertake any running, much less the ambitious course you describe. The most common theme on virtually all theads I have read on labral tears is the extent to which athletes like you and me, especially runners, ruin themselves continuing to pound and grind away after their tear(s) get started. I know it's gut-wrenching. I had to go through it myself January a year ago but if you don't stop you stand a good chance of winding up bed-ridden in considerable pain; even if you do stop working out the tear may still progress.

What more do you know about the extent of your injury?

Hi everyone,

I also wanted to talk about my recovery, since a lot of you seem to be having less than optimal results. I had surgery July 25th after being undiagnosed for a year and a half. This fall was a bit hard, especially as a college student, because I had to walk a lot and had some residual tightness in my adductors and hip flexors. I continued to bike, eliptical and swim all fall to stay in shape. I was cleared for track practice over Thanksgiving and have been making steady gains ever since. Last weekend I re-broke the school record in weight throw and am now ranked 17th in NCAA D3 in the event! There are weeks when my hip starts to bother me and I get worried that maybe the pain will come back. With help from the training room and some stretching I've been able to continue full workouts. It is strange to realize that my hip is not in pain most of the time. For 18 months I lived with constant hip pain caused by a 3 cm tear. As a 21 year old college athlete it was very hard to deal with the possibility of never getting better. I would absolutley reccomend the debridement surgery to anyone. It's not a quick fix!! I was under the impression that they'd take out the torn piece and a week later I'd be good to go. My recovery was quicker than most, but be patient. Good luck to everyone!!

Dear Coloradolora, I'm new to this Web site/discussion group. I just had an MR arthrogram that showed a small focus of labral detachment involving the anterior-superior aspect of the right hip labrum. The remainder of the labrum is normal. I've been having right hip/groin/ab/hamstring pain. I saw in an earlier post you mentioned that the worse scenerio was when the tissue gets pulled away from the hip bone, which sounds like it might be happening to me. I can't see my doctor for a follow up until next Wednesday. Do you know what's the difference between "detachment" and "tear?" I've been doing very light exercise in the last couple of weeks, just stationary bicycle and eliptical machine. I don't seem to be getting better. I live in Seattle and am not aware of doctors that specialize in this type of injury mentioned on the forum who live around here.

Carrie, I have a consultation with Dr. Kelly at the end of this month. He seems to be about as good as it gets in terms of reputation. I already know I have a labral tear in my left hip because I had surgery for Athletic Pubalgia (if you don't know what it is, it's basically a tearing of any musculature in the groin area) last summer with Dr William Meyers in Philadelphia and in my MR Arthrogram the tear showed up. I was advised to 'just leave it' unless the pain became less bearable. I was mostly fine this winter, playing ice hockey and otherwise keeping up my fitness routine, but then I felt it get markedly worse towards Spring and I feel like my next MRA will show it got worse.

Now that you're 4-5 months out from your surgery, how would you say you feel? Do you think, either from Dr. Kelly's advice or your recovery process, that you will return to "before" levels of activity and performance? Do you know if you are more or less susceptible to recurrent injury now that you've been fixed once? Does he think it's more worth it or less worth it to have surgery depending on your age? I'm 42 and probably a good candidate for surgery but I am concerned about being able to get back to my before fitness level without recurrent injury risk.

Hi Will,

I am a special case when it comes to this injury and have had recurrent issues. I have not re-torn the labrum at this point I don't think, but I had a severe case of synovitis in the joint during surgery and unfortunately even though I had a synovectomy done the synovitis has come back. Right now we are not sure why it has come back and we are doing all kinds of things to figure it out. I have contiued to have pretty bad pain and I cannot progress very far in PT cause I cannot do any strengthening exersises until I have no pain. I just had another steroid injection last week to help with the pain and so far it is helping somewhat. Unfortunately I cannot answer as to when I will be back to doing regular activities cause it seems a long way off for me. I am not unhappy that I did the surgery, I definitely needed it and we would have never found the synovitis without it, but now we have to find out why it keeps coming back. Dr. Kelly is a great doctor and he is doing all he can to figure out my condition. I wouldn't have it any other way or any other doctor at the head of my case. I actually have a few that he has included in this hunt for answers.
