My knees don't like the treadmill.  Need advice, please!

B/c of the weather, and having 3 kids ages 7 and under, I've resorted to using the TM at home sometimes, if I absolutely can't run outdoors. At first it was awkward, but I started to run at a slightly faster pace and on a 1% incline (I tried 0.5%, but that didn't feel right, either), and that felt a lot more natural for me.

 

BUT - after about 2 or 3 runs like this, I started to have excruciating muscle tightness kind of on the inside of my legs above my inner ankle (not really my shins, not my calf, not my ankle, but kind of where the 3 come together, for lack of a better way to describe it). I mean, the pain was so bad that I could barely walk the last time I got off the TM.

 

Today we were lucky, and the weather was nice (43* and sunny), so I took my 3 yr old in the jogging stroller, and got 3 miles in. My legs/knees feel great. But how can I get to where I can run on the TM if I can't go outdoors. There's going to come a point this winter where I just can't run outdoors b/c it'll end up being too icy/snowy.

 

I really need to figure out a way to run on the TM without having discomfort. My knees started bothering me when I ran without the incline, I think b/c my stride has to be "shorter" on the TM, or something. Not sure exactly what the problem is - my TM has a fairly long belt, so it's not like I'm gonna fall off the end.  It just doesn't feel natural to me, for some reason. The incline takes care of that, but then the constant incline hurts my legs (in an area that I can't really figure out how to stretch, at that).

The first thing I would try is to slow down. The 1% incline makes it equivalent to running outside, so the faster speed is just that - faster speed. The other thing I would look at is running tense. I know I tend to run less relaxed with the constraints of running on the treadmill - i.e., I have to concentrate on staying on the belt, a little bit at least.  Also check your form and footplant. Make sure you're running with your normal stride and planting your foot under your body, not out in front of your body.

Len pretty much touches all the points.  The only thing I can add is don't give up on the TM.  TM's are generally considered easier on your body than the roads, for equal speed, distance, and inclination.

 

Try starting at some fraction of your usual distance and build up.  Likewise with pace, try a slower pace and build up.

I know exactly how you feel...When I started running again this year back in May I strictly ran on my treadmill until just last month when I finally started taking my long runs outside, so I didn't have to stare at my walls and be on the treadmill for over an hour!! But, now that I have been outside, I find it so much more difficult to run on the TM!! I feel so confined, like I can't stretch out enough, or relax enough, and my runs are no longer even remotely enjoyable on the TM!! I have also noticed that my form and way I land are completely different then when I run outside!! If this is true for you I can see why you are having these pains in your muscles!! When I run on my TM, I always use at least a 2% incline, anything lower for me makes me feel like I'm going downhill, which is maybe contributing to your knee pain...I too, have a small child that keeps me from running outside during the week, so the TM it is for now, but weekends when my bigger kids aren't in school, I am so outside to run. My legs and entire body feel so much better outside!! Last Saturday I ran 10 miles outside, and felt great, then this week when I ran 4 miles on my TM it felt like it would never end, and I felt confined and my sides were cramped up the whole time. I hope it gets better for you...I guess maybe I would suggest, try upping the incline to 2%. Good luck!!!

Can anyone please help me?  I have the opposite problem - "my knees don't like the ROAD!!"  Asphalt causes medial left knee pain if I run exclusively on the road.  I NEED a break  (or my knees do) on the treadmill probably at LEAST 1x/week so I don't get injured.  Anyone else have this problem?  HELP!!

Do you have good shoes for running?  When you run on roads, the impact is generally greater.  If your shoes don't have adequate impact absorption and arch support, they could be the cause.

Thanks, Mr. Dave, for replying.  I wear ASICS 2130s which I like very much.  I'm hesitant to change them because they fixed my arch pain due to overpronation.  Guess I'll have to limit my roadwork to 2x/week, with the other 2x on the TM.  Thanks.

It seems to me that you should be able to run as much as you wish on roads. 

 

Have you ever had a gait analysis done in a running specialty store?  I've been impressed with the care and knowledge of the staffs at Fleet Feet and Roadrunner Sports shops.

 

Have you thought about your stride length?  You're not over-striding, are you?  (your front foot should hit the ground under or slightly behind your knee)

It could be that your treadmill isn't calibrated correctly and you are running at a faster pace than it indicates.  So try running slower.  The best way to judge is by comparing your breathing and heart rate to determine if your exertion on the treadmill is "harder" than your exertion outside.

 

 

I have noticed that different treadmills in the gym can sometimes  be significantly variable from each other, as much as 30-60 seconds per mile.

 

 

As the other posts mentioned, try to relax.  Lower your arms, don't hold your hands in a fist.  You don't need to pump your arms unless you're running 400s or less.  Just let your shoulders relax and swing naturally.

Are you by any chance running on "crowned" roads? Often asphalt roads slope downwards at the edges to drain away water when it rains. If you run for long periods on the same side of the road, you're effectively running with one short leg and one long leg, which can lead to injuries. If that applies to you, switching direction (from one side of the road to the other) periodically should help.

I had the crowned road problem myself so can vouch for that  being a real issue.  I had terrible pain in my groin (right where the top of the thigh meets the torso) and could barely walk without limping.  I refused to give in and stop running so I really tried to pay attention to what I was doing and read these posts like crazy searching for tips.  After a few weeks, I bought new running shoes, got extra arch support (cost a fortune but VERY worth it) and started running directly down the middle of the street (I run in the neighborhood in the very early am with almost no traffic so I can do that fairly safely with lots of reflective wear) and after about a week of doing this my pain was gone.

Even though I try to run on a flat surface it isn't always possible.  So I try to even out the discomfort when I have to run on a  crowned road (berm) by running out and back on the same side of the road.  That way I have one leg short on the way out then the other leg short on the way back home.  Of course that means I have to run with my back to traffic in one direction : {

OMG!!!!  My knees do the exact same thing!   I was fitted for running shoes and it helped some.   I still have more issues on the TM than outside.

If it doesn't hurt when running on an incline but does when it is flat....  my suggestion is to run on an incline.  This is pretty standard advice. That is, always run on a slight incline when on a treadmill.  It is unnatural to run with a moving surface.  The treadmill runs you so inclination minimizes that tendency. Keep it simple.
