Please complete registration form, attach USA Hockey Roster and $300 deposit.  No Team will be accepted into the tournament

without a 'cc' # on the regsitration form.

Final tournament payments are due by Jun 1, 2012.  Credit cards will automatically by charged for the outstanding balance on June 01st
